Hayley Graff

Hayley Graff, LMSW, currently serves as a Hospice Medical Social Worker at Amedisys since July 2024, helping hospice patients and their families address psychosocial and emotional needs at the end of life. Prior experience includes roles as an Engagement Specialist at Vital Decisions from July 2022 to July 2024, a Therapist Associate at Cerebral from June 2022 to November 2022, and a Hospice Social Worker at Heart of Hospice from August 2020 to May 2022. Hayley began the journey in social work as an MSW Intern at Heart of Hospice and also held positions as Graduate Assistant of Admissions at the University of South Carolina College of Social Work and MSW Intern at Alston Wilkes Society. Internship experience also includes work at Blue Ridge Court Services, supporting clients with bond conditions and resource connections. Educational credentials include a Master of Social Work from the University of South Carolina and a Bachelor of Social Work from James Madison University.
