How You'll Make a Difference:

The position of Social Services Case Manager will provide support and social services to health plan members and their families in accordance with an appropriate plan of care. The Social Services Case Manager provides in-home and telephonic assessment of needs, while continuing to develop, monitor, and evaluate care plans and progress, while facilitating interdisciplinary approaches to social needs.

  

Essential Duties and Responsibilities:

  • Schedule and complete face to face visits with patients/families/caregivers
  • Complete assessment for social service needs to address barriers to health care
  • Assist and provide resources, information, and referrals to support the care plans
  • Participate in interdisciplinary team (ICT) meeting to establish, review, and revise the patient care plan
  • Maintain electronic medical records
  • Support patient, family, and caregivers with cultural and spiritual sensitivity
  • Provide holistic, family-centered care across treatment settings to improve the quality of life
  • Provide supervision and guidance to the member support specialist team
  • This person will travel to patients home in the valley up to 80% of the time

Qualifications and Skills:

  • Degree in Social Services or related field/ or equivalent experience

  • Maintain ongoing Social Services training to stay abreast of changes in the community and educational resources for members

  • Current BLS/CPR certification required

  • Must have valid driver license and appropriate liability insurance

  • Current TB test and maintain a negative test annually required

  • Hep B vax/titer proof (Company does offer/or may sign a declination) required

  • Current flu shot (or sign waiver/follow mask policy) required

  • In-depth knowledge of health care services and other medical terminology is required
