Job purpose

The Referral Coordinator is an integral part of Heartland’s clinical team who is responsible to coordinate all patient referrals at the request of the providers. Will schedule all appointments necessary, coordinating with the patient and the outside Providers. Will notify patient of said appointment and give any instructions if needed. Will enter all referrals into the eCW system, will track and record reports as they are received.

Duties and responsibilities

  • Track and coordinate all patient referrals and radiology orders

  • Schedule patients for referral appointments and radiology appointments as needed

  • Notify patient of all upcoming appointments and any instructions, if needed

  • Attach report to referral after appointment date

  • Follow up with patient/facility if report is not received after appointment date

  • Fax clinical information or medical records to outside facilities as indicated

  • Assist with scanning documents into the patient’s chart and assign to appropriate provider

  • Attend required meetings and trainings as assigned

  • Other related duties or responsibilities the clinical staff or supervisor may assign when need arises

Qualifications

  • Preferred medical office experience, knowledge of medical terminology and HIPAA

  • Ability to work with clinical staff to handle pressure from multiple patients, staff, and providers

  • Commitment to the mission of Heartland

  • Exhibits the ability to manage time well and work under stressful conditions with an even temperament

  • Displays ability to establish and maintain harmonious working relationships with other employees and the public

  • Demonstrates ability to understand and follow oral and written instructions

  • Ability to learn about patients and their challenges

  • Must have the ability to address sensitive issues in a confidential manner

  • Must have basic computer skills including word processing, basic knowledge of office procedures, and business machines

  • High school diploma or equivalent

  • Job tasks are performed in close physical proximity to other people

Physical requirements

  • Requires the ability to work at a desk and computer for extended periods of time

  • Requires occasional light lifting (under 10lbs)

  • Requires using hands to handle, control, or feel objects, tools, or controls

  • Requires standing and walking

Details

  • Full time job that averages 40 hours per week, Monday through Friday as scheduled.
    • Hours are generally between 8am-5pm as scheduled and may be altered by supervisor(s) as needed
