Anne D. Constantino

Anne was appointed to her current position as President and Chief Executive Officer for the Horizon Corporations in 1994. In this role she leads the development and implementation of strategic and annual operating plans for Horizon Health Services, Horizon Village and Health Management Group. During her tenure, Anne has led the organization in expansion of services both geographically and clinically in Western New York. With a commitment to corporate culture that is conducive to excellence, the organization has been designated as a NYS Best Place to Work for the past 12 years.

A graduate of Buffalo State College and SUNY at Buffalo, Anne received her M.S. in Educational Psychology and Rehabilitation Counseling. Additionally, she is a graduate of Leadership Buffalo, Class of 2003. In 2013, Anne was recognized by the Buffalo News as an Outstanding Citizen of the Year.

Anne is the Board President for the NYS Association of Substance Abuse Providers and serves on the executive boards of the NYS Coalition of Community Agencies and Millennium Collaborative Care (DSRIP/PPS). Anne is a founding member and partner of the Value Network, IPA, LLC . In 2017, Anne was appointed to the Governor’s Task Force on Opiate Addiction. Anne also serves on the Foundation Board for Buffalo State College.
