ACT is a broad, multistakeholder collaborative initiative to support and advance care transformation efforts and Learning Health Systems using proven frameworks, methods and tools. Building on 2 decades of work funded by many public and private organizations, ACT was launched in 2023 with support from the VA. ACT is currently managed by TMIT Consulting, LLC and guided by a multi-stakeholder Steering Committee including representatives from health systems, public health agencies, standards organizations, patient partners and other key groups.
