In addition, they increased enrollment and improved performance in Medicare Advantage chronic condition special needs plans (C-SNPs) across 12 US geographies in multiple states (decreased hospitalizations 24% and mortality 25% vs. standard care with $10,000/patient/year saved).

Looking to scale effective, low cost, care coordination programs, Bryan and their team created compliant new incentive programs for physicians and a new national NP practice to align their efforts with community physicians (total physician number > 1100) in managing high-risk patients and smooth transitions of care (reduced readmission rate by 22%). Data has also been used effectively during Bryan's time at DaVita Kidney Care with clinical and claims analytics informing a proprietary risk model and helping guide process and clinical action for high-risk, high-need patients.

Bryan Becker went to Dartmouth College for their AB in English, then to Tulane University for their MMM in Medical Management, and finally to the University of Kansas Medical Center for their MD in Medicine.
