Jen Hand, MS, CDP, CADDCT, currently serves as a Transitional Care Manager at BAYADA Home Health Care since January 2026, following a position as an Elder Care Coordinator at Bratton Estate & Elder Care Attorneys from June 2021 to December 2025. Before that, Jen worked as a Helpline Coordinator and Care Consultant at the Alzheimer's Association®, where responsibilities included managing the daily operations of the Helpline, training volunteers, and providing educational support to families dealing with Alzheimer's disease.
