BRIDGE To Home is dedicated to providing safe and essential care in the home for individuals of all ages with disabilities, special healthcare needs, and/or mental health needs.  Our direct support professionals (DSPs) are experienced, thoughtful, and well-trained.  We are here to create an individualized care plan that is right for you and your family.  We help people with safe companionship navigate daily activities at home and out in the community.



We also recognize the enormous impact of chronic caregiving.  Balancing work and other family responsibilities with chronic caregiving for a loved one can be challenging.  It can lead to exhaustion and little time for self-care.  We can help.
