Carlette Johnson, CHW, BS, CNP, has been a Community Health Worker at Baylor Scott & White Health since December 2010, specializing in coordinating health teaching classes, performing biometrics, and promoting health improvements through nutrition to prevent chronic diseases. As a Certified Community Health Worker, Carlette served as a Patient Navigator from 2016 to 2017 and currently leads the DHWI Farm Stand, managing volunteers from senior adults to youth.
