The Patient Care Team at CareFirstNY provides comprehensive support to individuals and families facing serious illness or end-of-life. This multidisciplinary team, including a Palliative Care Nurse, Clinical Therapist, Hospice LMSW, Patient Care Volunteer, and Registered Nurse, collaborates to deliver personalized palliative and hospice care, mental health counseling, and grief support to ensure the physical, emotional, and spiritual well-being of patients and their loved ones.
