This team includes roles such as the Director Of Client Services Acute, Admissions Counselor, Intake Specialist, Community Liaison, Discharge Planner, and Utilization Review Specialist. They collectively manage patient admissions, coordinate care plans, liaise with community resources, and handle discharge processes while ensuring utilization of services is efficient and aligns with patient needs.
