Centric Physicians is a Medical practice providing healthcare to patients in assisted living and independent living communities, skilled nursing and long term care facilities, acute rehab facilities, long term acute-care hospitals, and acute care hospitals. We are a full spectrum practice focusing primarily on the care of patients age 45 and above. Our providers are recognized as experts in treating the complex medical needs of this population. Centric Physicians has developed a proven comprehensive post-acute care with a full behavior health division as well as a transitional care management program to address and correct the hazards, inefficiencies, and over-utilization of medical resources that exist in today's fragmented healthcare system. We do this with a combination of facility score-carding, leveraging of solid evidence-based clinical guidelines and protocols, and developing treatment plans with patients and families to assure that the necessary and appropriate medical care is provided to our patients at the right time and in the right location.
