David Tavarez is the Director of QA/QI at CHDFS, Inc., where they are involved in outreach, quality assurance, and quality improvement for Medicaid provided supportive services for members and families in need. Prior to this role, they served as the Outreach/Quality Assurance Coordinator for Health Homes and worked as a College Assistant at The Urban Male Leadership Program at Lehman College from 2013 to 2017. David earned a Bachelor’s Degree in Business Administration and Management from the City University of New York-Herbert H. Lehman College in 2014 and graduated with honors from the Urban Peace Academy in 2009.
