ChronicCareIQ was founded by Matt Ethington in 2015. He was diagnosed with Type 1 Diabetes at the late age of 30, and personally experienced the disconnect and frustration experienced by both physicians and patients alike in the treatment of chronic disease.    

ChronicCareIQ is award winning technology that enables physicians and hospitals to keep better tabs on chronic and fragile patients while simultaneously tracking and documenting all care management reimbursements.

CCIQ is an  early warning system to know when fragile and chronic patients require your attention. Patients respond to automated, algorithm driven electronic queries via smart phone app, text, email, desktop or tablet. Queries match patient health profiles and are dynamic to detect disease progression, decompensation, change in symptoms and patient status. Those, whose responses indicate they are trending poorly or who have crossed clinical thresholds are called to attention on a color-coded dashboard for the physician to follow up.
