Clearfield Advisory works with organizations preparing a second AI attempt — those who experienced firsthand that the first failure was structural, not technical, and need to address the infrastructure before the next deployment begins.

95% of enterprise AI pilots fail to deliver measurable financial return. 42% of organizations abandoned most of their AI initiatives in 2025 alone.

The organizations now preparing a second attempt face a narrowing window — and a question nobody has given them a precise answer to:
What would need to be true about this organization for the second attempt to produce a different result than the first?

The answer is not a better technology platform. Not a more experienced change management firm. Not a more sophisticated governance framework. The answer is sound Human Decision Infrastructure — the five structural conditions that determine whether any implementation can hold.

Those five conditions are Authority, Orientation, Risk, Temporality, and Agency. Together they form AORTA — the infrastructure that every organization is either building, maintaining, or slowly losing.

When the infrastructure is sound, AI multiplies organizational capacity. When it is fractured, AI multiplies organizational dysfunction. The technology performs precisely as designed in both cases.

Clearfield maps the fracture. The repair belongs to the organization.
