Lisa Simmons-Fields, DNP, is currently the Director of System Population Health and Care Management at Trinity Health in Michigan, a role they have held since 2018. With a strong focus on population health, care management, and integrated care delivery, Lisa has also served as President of CMSA Detroit and is set to become the National Director for CMSA. They previously managed care management teams at St. John Hospital & Medical Center and St. John Providence, where they developed integrated clinical care management models. Lisa earned a Doctorate in Nursing Practice with a concentration in Population Health Management from Madonna University.
