Susan Mojaverian

Susan Mojaverian, MSN, RN, is the owner of ComForcare Home Care, where they focus on promoting optimal outcomes for seniors aging in place in Chester County, PA. With extensive experience in home care services, Susan has worked to support families managing complex needs and has held roles such as Clinical Assistant Professor of Nursing at Drexel University and Hospice Case Manager at Montgomery Hospital. They have also contributed to the Community Care Coalition of Chester County to enhance patient care transitions and reduce hospital readmissions. Currently, Susan is pursuing a BA in Psychology and a Master of Science in Nursing, Community Health.
