CommuniCare Advantage is a caring and professionally run provider-owned Medicare Advantage–Prescription Drug (MA-PD) plan. A partner of CommuniCare Health Services, which is a leader in post-acute care, CommuniCare Advantage (CCA) provides accessible and affordable health insurance for special needs populations as defined by CMS. CCA offers two plans: 
• An Institutional Special Needs Plan (ISNP) for qualifying residents of participating nursing facilities 
• A Chronic Condition Special Needs Plan (CSNP) for eligible individuals with diabetes mellitus, chronic heart failure, and/or cardiovascular disease
These plans design services and benefits around the unique health needs of these special populations. 

CCA is based in Ohio and currently serves selected counties in Indiana, Maryland, and Ohio.
