Community Health Net (CHN) is a Federally Qualified Health Center (FQHC). We are multifaceted organization, providing healthcare dental care, vision care and other related healthcare services.  We serve the homeless, economically disadvantaged, persons with HIV or AIDS, and anyone who needs healthcare services.  We here at CHN ensure access to healthcare services for all individuals, regardless of insurance status or ability to pay.  We do that by offering sliding fee discounts based on family size and income and nominal fees. Our Mission is to improve the quality of life in our region, by providing professional healthcare services with compassion, respect and dignity to all.
