Community Support Initiative is a CalAIM provider and nonprofit organization that offers critical health and social support services to vulnerable populations. We  specialize in addressing the complex intersection of healthcare needs and social determinants of health through housing support, medical case management, and post-hospitalization services. Serving adults, families, and youths experiencing homelessness, justice-involved individuals, individuals with mental health needs, and others, we work to create strong relationships and deliver more integrated, person-centered care.
