At Connectus Health, we are dedicated to providing affordable, high quality healthcare that is designed around you and your healthcare needs.  Connectus utilizes the Patient-Centered Medical Home (PCMH) model of care. A Patient-Centered Medical Home allows you to personalize your care plans, supports better understanding and monitoring of prescription medications, provides tailored healthcare coaching and advice, and creates stronger relationships with your healthcare team and other community resources.
