Providing people living with disability the opportunity to embrace activities and experiences enabling them to build their capacity, flourish and enrich their lives. 
Our service is based on a community-based model where we aim to connect participants to their local community. We are excited to bring life to day services, as we have seen they are often overcrowded, and the quality of service has gone missing.
We hope to change this around by using a personalized approach and engaging with allied health professionals to ensure recommendations are implemented.
At Creative community Pathways we have a wealth of experience in disability, ranging from behaviour support, residential support, direct support to Support coordination.
We understand the challenges the NDIS space brings to everyday life, and we are open to work with you to bring positive change in your life and you family- we believe communication is key!
