El Centro de Corazón (El Centro) is a Federally Qualified Health Center (FQHC) and 501(c)(3) non-profit health care organization that provides adult and pediatric primary care, women’s health services, dental care, and behavioral health services through its health center locations.

The Outreach and Community Health team at El Centro de Corazón focuses on engaging and educating the community about available health services, enrolling individuals in health programs, and promoting preventative health practices. The team consists of a Community Health Worker Manager, Outreach Enrollment Specialist, and Outreach Coordinator/Community Health Worker, working together to improve access to quality health care and enhance the overall well-being of the populations served.
