Evergreen Mentorship is a mission-driven telehealth provider dedicated to leveling the playing field for marginalized community members across Ohio. We provide high-quality mental health case management and counseling exclusively for adults (18+) covered by Medicaid.

Our 100% remote model removes traditional barriers to care—such as transportation and scheduling—allowing our clients to receive life-changing support from the safety and comfort of their own homes. We are more than just a service provider; we are a community of advocates. Our team of Qualified Behavioral Health Specialists (QBHS) and Licensed Clinicians work one-on-one with clients to manage stress, navigate life transitions, and achieve personal, career, and educational goals.
