Feltin’s Approach to Primary Care:
We deliver comprehensive person-centered primary care services to people with complex medical needs in their home. Our mission is to enable autonomy and support independence, to acknowledge and honor the uniqueness of each individual in our care. To that end, our care plans prioritize quality of life, dignity of risk, and patient goals while minimizing disruption and dislocation
as much as possible. We schedule follow-up visits on a regular basis and address new issues as they arise to mitigate the need for urgent care, ER visits, and hospitalization whenever possible. We place a heavy emphasis on care coordination, interdisciplinary teamwork, communication, and collaboration with all members of a patient’s care team. We are committed to expanding access to meaningful primary care for people who have serious difficulty in getting care within traditional models of care.
