Fern Care Hub is a small, local NDIS provider based in Knox, South East Melbourne.
We exist for one simple reason: too many young adults (18–35) with disability were missing out on real social connection and everyday life experiences. We’re here to change that.
Using local neighbourhood houses and community venues, we offer relaxed, inclusive group programs designed specifically for young people with intellectual disability, autism spectrum disorder and psychosocial disability who have low-to-moderate support needs.
Our programs include: • Group Vibes – small group activities
• Active Citizen – community outings across Knox
• Skill Builders – practical independent living skills
Everything is delivered in small groups with a warm, supportive and non-clinical feel. We believe real growth happens through genuine connection, not clinical environments.
Currently accepting self-managed and plan-managed NDIS participants.
