Jake Young

Jake Young is the CEO and Founder of FormDr, a company that provides practices with the tools to send and receive patient forms online. Jake has been in this role since March 2018. Prior to that, Jake founded Siteplicity in September 2012, a web design company aimed at simplifying the web design experience for small businesses. Jake successfully grew the company to over 1000 sites before selling it in February 2019.

Jake Young attended The Art Institutes from 2004 to 2008, where they obtained a Bachelor of Fine Arts (BFA) degree in Interactive Media Design. Jake also obtained a certification in General Assembly's Javascript Development from General Assembly San Francisco in 2016.
