Innovative Integrated Health provides elderly individuals with health care services in the privacy of their homes through Fresno’s Program of All-Inclusive Care for the Elderly (PACE)—a unique healthcare program that offers medical, health and wellness services, and more to seniors who have chronic illness or disabilities.

We serve seniors who:

Are 55 years old and above
Reside in Fresno County
Require assistance with activities of daily living
Need a level of care equal to nursing home services
Can live safely in the community with the help of PACE services
It is our goal to assist and support our clients so they can remain at home and in the community with independence and dignity through our wide range of medical and social services. We also aim to contribute to improving the healthcare system by eliminating disparities in health care access and outcomes for the multi-ethnic and underserved senior communities in Fresno County.

It has come to our attention that third parties outside of our organization have created domains and emails (known as domain/email spoofing) that appear to come from Innovative Integrated Health, Inc. (innovativeih.com) and our affiliates, Fresno PACE (fresnopace.org), Bakersfield PACE (bakersfieldpace.org) and Orange County PACE (ocpace.org). We ask everyone to act with extreme vigilance.
