Heidi Young

Heidi Young, MD, FAAHPM is a physician leader currently serving as Regional Medical Director at HarmonyCares, focusing on delivering home-based primary care to patients with complex medical needs. Previously, they held multiple leadership roles at Capital Caring Health, including Associate Chief Medical Officer and Hospice Medical Director, where they developed innovative clinical programs. Heidi also served as Medical Director at Advanced Illness Partners, emphasizing value-based care principles. Their earlier experience includes being Section Chief of Palliative Medicine at MedStar Georgetown University Hospital and an Associate Professor at Georgetown University School of Medicine, where they taught communication skills and symptom management. With over a decade of expertise in internal medicine, palliative care, and hospice services, Heidi is dedicated to improving outcomes for diverse patient populations.
