Heritage Health Network (HHN) is a California-based Enhanced Care Management (ECM) and Community Supports provider serving Medi-Cal members with complex health and social needs.

As part of California’s CalAIM initiative, ECM is a Medi-Cal benefit that provides intensive, whole-person care coordination for individuals who need extra support navigating medical, behavioral health, and community services. Community Supports extend care beyond the clinic by addressing needs such as housing, nutrition, recuperative care, and home safety.

At HHN, we partner with managed care plans, hospitals, primary care providers, and community-based organizations to ensure members receive coordinated, accountable care across settings.

We focus on populations experiencing repeated emergency department use, housing instability, high-risk chronic conditions, and other complex challenges.
