Gary Jacobs

Gary is President of Village@Home, an extension of the physician-led, team-based platform developed by VillageMD, a national provider of primary care. He has experience identifying new business opportunities, developing long-term growth strategies and strategic alliances, and implementing enterprise-wide initiatives for health plans and post-acute and long-term care providers.

Prior to joining Village@Home, Gary was the Managing Director of Circumference Consulting, a boutique consulting firm specializing in helping post-acute and long-term care providers succeed in the value-based payment world and assisting payers to initiate value-base payment models. Gary also served as Executive Vice President for strategic relationships for CareCentrix and was a Managing Director in the Heath Industries Advisory Practice at PricewaterhouseCoopers (PwC), leading the National Government Programs practice.

Gary earned his bachelor’s from Boston College and a Master’s in Public Administration from American University. He is a board member for National Hispanic Council on Aging and the Visiting Nurse Association.
