MĪNA strives to create an accurate, complete and simplified health history form that can be wirelessly and efficiently scanned and incorporated into your routine workflow. This information can be uploaded to the EMR, left on the computer for reference during dictation or printed to incorporate into the physical chart.  This will encourage and empower the patient to develop and maintain an accurate medical history to communicate with you, the healthcare provider.  Intuitively this will decrease the number of medical errors including, but not limited to, diagnosis omission and medication errors, which will help improve the accuracy of medical reconciliation.  Finally, this has been built to aid in accurate documentation for individuals with language barriers.
