Our Chief Medical Officer leads a clinical team of licensed nurses that leverages proprietary health IT technologies, tools and informatics to provide timely and targeted care for high-risk patients with chronic diseases and preventive care for lower-risk patients, which reduces more costly and avoidable condition deterioration, hospital admissions and emergency room visits. 

Our provider network is comprised of more than 650 primary care physicians, multi-specialty physicians and other healthcare professionals, located primarily in 14 of the 21 counties in Central and Southern New Jersey.

We are a privately-held corporation owned by 47 individual shareholders all of whom are primary care and specialty physicians.
