Rachel L. serves as the Lead Clinical Case Manager at PATH, focusing on the HUD-VASH program to support unhoused Veterans through clinical mental health care and housing solutions. Previously, Rachel worked as a Medical Social Worker at Haven Health, providing psychosocial support and utilizing evidence-based therapies. Experience includes serving as a Mental Health Counselor at Nsight Mental Health and Addiction, where group therapy and crisis intervention were key responsibilities, and as a Medical Social Worker intern in hospice at PROVIDENCE ST JOSEPH HOSPITAL. Additional roles include Social Researcher at the University of Southern California and Account Manager at Triple Crown, along with a background in nutrition and education as a Recovery Counselor and Tutor. Rachel holds a Master's degree in Social Work from the University of Southern California and a Bachelor's degree in Nutritional Science from San José State University.

We strive to do this by prioritizing housing while providing customized supportive services for people in need. Our agencies each address the homelessness issue in a different way—supportive services, permanent housing development, support for homeless families, and community engagement—all of which ultimately help the people we serve Make It Home.
