One-fifth of deaths are avoidable each year, yet there is no national system to collate and learn from them. The Preventable Deaths Tracker (PDT) was created to change this. 
  
Founded and led by Dr Georgia Richards at King’s College London, the PDT's core mission is to save lives by enabling the learning and prevention of avoidable and premature deaths through evidence-based vigilance.
