Sanjhi Sehat is a rural health initiative working to make primary healthcare understandable, reachable, and truly accessible for families across Punjab.

While we strengthen primary care access, our core mission is to tackle the growing burden of Non-Communicable Diseases especially Diabetes and Hypertension through early detection, continuous counselling, and strong community engagement.

In many villages, care is delayed not because services don’t exist, but because people lack clear, practical information about the system. Families often don’t know when and where the right doctor is available, what type and quality of medicines or supplies the Govt health centre provides, where routine tests can be done, or which symptoms require immediate attention.
This confusion leads to late diagnosis, avoidable expenses, and complications that could have been prevented.

Sanjhi Sehat was created to bridge this critical gap through Continuity of Care Model

Our work includes:
* Early screening for Diabetes & Hypertension
* In-person and tele-counselling for long-term treatment adherence
* Strengthening community awareness and risk understanding
* Guiding families on symptoms, care pathways, and government services
* Improving access to dependable primary care through field outreach and digital support

We are currently working in Ajnala and Harsha Chhina (Amritsar district) under the Aspirational Block Programme of NITI Aayog, with a clear goal:
Make timely, affordable, and reliable care accessible for every rural household regardless of income, education, or geography. At Sanjhi Sehat, we believe health becomes equitable only when information is clear and care is continuous.
