Solomon Medical is a premier post acute and long-term care (PALTC) practice building exceptional healthcare teams.

Founded by Dr. Matthew “Joel” Awerbuck, Solomon Medical provides clinical supervisory services to skilled nursing facilities (SNFs) across Northern California. We work exclusively in the PALTC setting. Our board-certified internal medicine physicians and highly-trained nurse practitioners serve as liaisons between hospital discharge staff, patients' families, and ancillary clinicians. With a coordinated continuum of care, patients experience smoother handoffs and achieve better outcomes.
