Southeast Health Group is a nationally recognized community health organization serving six rural counties in southeastern Colorado. 

We provide comprehensive health care services out of 11 offices, including primary care, mental health care, and substance use treatment.

We have special funding for our returning soldiers and their family members through our Stationed Home program.
