Thrive365 is an NDIS Registered Provider supporting adults across Western Sydney and South-East Queensland with high-quality SIL, SDA bridging, MTA, dementia-specific care, Community Access, and Day Programs.

We exist to create safe homes, skilled support, and meaningful days for every person we serve. Our focus is simple: provide the right care, in the right environment, at the right time. Support Coordinators choose us because we act fast, communicate clearly, and prioritise stability and wellbeing in every placement.

Our team is trained in complex clinical care, behaviours of concern, and dementia support, with properties and staff guided by Dementia Australia. We support participants with high physical needs, cognitive decline, mental health conditions, mobility challenges, and complex behavioural profiles.

What sets us apart is the way we integrate clinical understanding with genuine human connection. Our homes are warm, structured, and well-maintained. Our staff embody respect, consistency, and compassion — creating environments where people feel safe, valued, and supported.

Our Community Access and Day Programs are built around real interests and meaningful experiences. From skill-building and social connection to outdoor activities, festivals, and creative programs, we design every activity to help participants build confidence, engage with community life, and work toward their NDIS goals.
