UCare is a community-based, nonprofit, growing health plan providing health coverage and services across Minnesota and parts of Wisconsin. Working in partnership with health care providers and community organizations, UCare offers: 
•	Medicare Advantage plans across Minnesota and parts of Wisconsin
•	Minnesota Senior Health Options (MSHO) for people age 65 and older who have both Medicare and Medical Assistance
•       Institutional Special Needs Plans (I-SNPs)
•	Individual and family health plans, including plans with M Health Fairview and North Memorial Health, available through MNsure 
•	Minnesota Senior Care Plus (MSC+) for people age 65 and older on Medical Assistance
•	UCare Connect plans for adults with disabilities (physical, development, mental)
•	UCare Connect + Medicare for adults with disabilities who have both Medicare and Medical Assistance 
•	EssentiaCare Medicare Advantage partnership plans with Essentia Health in northern Minnesota and parts of Wisconsin
•	M Health Fairview & North Memorial Medicare Advantage partnership plans in the metro
•	MinnesotaCare and Prepaid Medical Assistance Program – Income-based programs for individuals and families
