At UHN Connected Care, we break down barriers to change, connecting people who want to solve healthcare problems. With 10+ years’ experience in delivering sustainable change programs, we have garnered strong national, provincial and regional knowledge. Our work includes implementing digital health assets for Ontario, such as a GTA West Diagnostic Repository, a Resource Matching and Referral system, a Critical Care Information System and ConnectingGTA, the foundation for a provincial e-health records system. In addition to our integrated project and change management know-how, we are adept at working through policy for information management, privacy and security.
