Union Community Care has purposefully connected with our communities throughout the COVID-19 pandemic. Now, we set out on the long journey of supporting patients recovering from COVID-19, vaccinating the hard-to-reach and vulnerable among us, and helping families and communities heal emotionally and physically.

We understand and embrace the complex lives and unique strengths of our patients, and we work hard to break down all barriers to care. This means we look through a grassroots lens. We go right into our community because we are our community. We are all a neighbor, a friend, a family member, and collectively, a trusted health home.
