When Oxygen Alliance was founded in 2021, we set out to address a critical challenge at the height of the COVID-19 pandemic: non-functional medical oxygen systems in low- and middle-income countries (LMICs). These essential yet idle devices were contributing to preventable loss of life. As this work progressed, it became clear that oxygen equipment failures were not the root problem, but rather a visible symptom of a much broader systemic issue. Across many settings, up to 70% of biomedical equipment was non-functional, driven by limited maintenance capacity, shortages of spare parts, insufficiently trained biomedical personnel, and competing national priorities.
