TrustedCare, has developed a personalized risk analytics and care management approach to address member’s clinical and non-clinical wellness challenges for members with chronic Cardiac, Pulmonary, Metabolic and ESRD co-morbidities. With this capability TrustedCare, facilitates providers and care management team in the creation and orchestration of member specific comprehensive care plans. In addition, working with its care network partners TrustedCare assist in addressing specific care and wellness needs. TrustedCare conducted a pilot with a MA plan and demonstrated its platforms capability to identify 20 percent improvement in the cost and wellness of their population.
