Zumbro Valley Health Center is a private non-profit organization that provides mental health, substance use disorder, primary care, and community-based services to twelve counties throughout southeastern Minnesota. We believe that improving our clients’ mental health outcomes is not achieved by a single person or program, but rather it’s a person-centric, coordinated approach to their care. Our professional staff works together with clients and others in the community to develop recovery-oriented treatment plans around each client’s distinct goals. Our person-centric, coordinated care approach includes a range of services that focuses on improving our clients’ overall health. 

Using an integrated model of care, we share information among team members relating to client care and establish a comprehensive treatment plan to address the physical, psychological, and social needs of each individual client. Each health care team includes a diverse group of members such as physicians, nurses, psychologists, therapists and other health professionals – all focused on the needs of the client. Our goal is to equip people with the tools necessary to create the healthy, rewarding life that supports their personal values.
