Crystal Koontz

Crystal Koontz is a Licensed Practical Nurse with extensive experience in various healthcare settings. Currently, Crystal works at VITAS Healthcare since October 2020, focusing on palliative care. Crystal has also been with Advantage Nursing Services in Pediatric Home Care since September 2013. Previous roles include LPN positions at HCR ManorCare and Riverview Assisted Living, as well as serving as a Health Coach LPN at OSF Medical Group, where the focus was on complex care management for pediatrics. Early in Crystal's career, experience was gained as a Hospice Nurse at Helping Hands Healthcare Inc. Educational background includes a degree in Licensed Practical/Vocational Nurse Training from Illinois Central College and attendance at Peoria Notre Dame from 1994 to 1998.
