Dale Lilburn

Board Of Directors at Aging Care Connections

Dale Lilburn has extensive experience in leadership and governance, currently serving on the Board of Directors for Aging Care Connections since April 2017 and as a member of the Board of Advisors for the La Grange Area Department of Special Education since May 2016. Prior to these roles, Dale Lilburn was the Chief Executive Officer of Plymouth Place from January 1999 until retirement in December 2017. Educational qualifications include a degree in Public Administration with a focus on Business and Political Science from the University of Missouri-Columbia, completed between 1968 and 1972, as well as a high school diploma from Bayless High School, earned in 1967.

Aging Care Connections

Aging Care Connections is a leader in providing client-centered care to older adults in Chicago’s west suburban townships. Aging Care Connections is also a nationally recognized leader in bridging the path between the hospital and homes for older adults. Based in La Grange, IL, Aging Care Connections is a non-profit organization that serves over 8,000 older adults, caregivers and their families from 38 communities in the Lyons, Proviso, Riverside, Leyden and Norwood Park townships. The mission of Aging Care Connections is to enrich the quality of life in our community by providing older adults and their families with guidance and support that enhances the ability to age well. Since first opening its doors in 1971, Aging Care Connections has grown and developed to offer a comprehensive range of programs and services specifically designed to meet the changing needs of our community’s aging population. Professional staff provides clients with community-based services using a social work lens, links clients with other community organizations as needed, and works to build a culture of health in our service area. Our programs include: Adult Protective Services, Transitional Care, Aging Well Neighborhood, Benefits Assistance, Caregiver Support, Comprehensive Care Coordination, Congregate Meals, Special Diet Home Delivered Meals, and Information and Assistance.
