Rayhaan Adams

Rayhaan Adams, MSW, LCSW, currently serves as Senior Director of Affordability and Senior Director of Wellness & Attribution Performance at Aledade, Inc., a role held since September 2022. Prior to this, Rayhaan spent nearly a decade at UNC Health, progressing from Care Manager and Population Health Leader to Director of Population Health Clinical Services, where responsibilities included overseeing primary care operations and implementing population health strategies. Rayhaan's early career involved managing a population health team and supervising MSW students at the UNC School of Social Work. Educational achievements include a Master of Social Work (MSW) focused on Clinical/Medical Social Work and a Bachelor's degree in Psychology, both from the University of North Carolina at Chapel Hill.
