Hazel Clements Ma

Hazel Clements, MA, PMP has a diverse work experience spanning over two decades. Hazel currently serves as the Vice President of Population Health at AllCare Health, where they have held various roles since joining in 2021, including Senior Director of Population Health and Director of Care Coordination.

Prior to their current position, Hazel worked as the Birch Grove Collaboration Director at Birch Grove Collaboration from 2016 to 2021. In this role, they facilitated systems change within partner agencies to improve the integration of physical and behavioral health care for high-risk patients.

Hazel obtained a Master of Arts (MA) degree in Audiology/Audiologist and Speech-Language Pathology/Pathologist from Western Washington University, graduating in 1998. Prior to that, they completed their Bachelor of Arts (B.A.) in Speech Pathology/Audiology at the same university, graduating in 1996.

In addition to their formal degrees, Hazel has pursued various certifications and professional development courses. Hazel obtained their Project Management Professional (PMP) certification from the Project Management Institute in April 2022.
