Azam Farooqui

Azam Farooqui, MD, currently serves as the Quality Measure Oversight Subcommittee Liaison and a member of the Committee on Practice at the American Society of Hematology, where responsibilities include coordinating quality measures to enhance hematology care. As the Hematology & Oncology Subspecialty Director for the Residency Program at Dignity Health and a Hematology/Oncology Physician at Ironwood Cancer and Research Centers, Azam Farooqui demonstrates a commitment to patient care and education. Previous experience includes a Hematology/Oncology Fellowship at SSM Health Saint Louis University Hospital and an Internal Medicine Residency at the Indiana University School of Medicine. Education credentials include an MD from Rush University and a BS in Community Health from the University of Illinois Urbana-Champaign. Research experience includes work as a Research Assistant in the Oncology Department at the University of Chicago Medical Center.
